notice_summary_english: The subject of the order is "Successive deliveries of dressing and operating materials, specialist dressings for the Hospital Pharmacy of the Provincial Hospital in Kielce".
A detailed description of the subject of the order and the quantity is specified in Annex No. 2 to the SWZ - assortment and price form. Information regarding the principles of delivery, warranty and remuneration includes: attachments:
• Annex No. 3 - draft provisions of the succession contract regarding public procurement,
• Annex No. 3a to the SWZ - draft provisions of the public procurement contract regarding the subject of lending, applies to Package No. 19 and 20.
Medical devices constituting the subject of the order should be understood as medical devices within the meaning of the Act of April 7, 2022 on medical devices, having the necessary documents authorizing marketing and use in the territory of the Republic of Poland.
The Ordering Party allows the possibility of submitting partial offers for individual packages, none of the packages can be divided:
Package 1 - Operating drapes, setons, tuppers, compresses: neurosurgical, sterile, non-sterile, with X-ray thread, gauze and non-woven
Package 2 - Non-sterile compresses and gauze
Package 3 - Sterile treatment packages
Package 4 - Headbands, plaster bases, mesh, cotton wool, lignin
Package 5 - Headbands
Package 6 - Plaster bands
Package 7 - Rhinological tampon
Package 8 - Set of waterproof patches
Package 9 - Wound healing preparation
Package 10 - Sterile tupfers II
Package 11 - Specialized dressings for difficult-to-heal wounds I
Package 12 - Specialized dressings for difficult-to-heal wounds II
Package 13 - Specialized dressings for difficult-to-heal wounds III
Package 14 - Specialized dressings for difficult-to-heal wounds IV
