II.1) DESCRIPTION

UK-Torfaen: the provision of specialist placements for individuals with
complex care needs.

Newport, Monmouthshire and Torfaen.

II.1.5) Short description of the contract or purchase(s): Torfaen
,Blaenau Gwent, Caerphilly, Monmouthshire, and Newport Local Health Boards
are inviting established and experienced organisations to express their
interest in the provision of specialist placements for individuals with
complex care needs including Acquired Brain Injury and Huntington's
Chorea.
The objective is to secure the provision of expert individualised care for
individuals with highly complex healthcare needs that require treatment in
conditions of specialist placements. Clinical and social input will be
provided by a skilled multi-disciplinary team, with a therapeutic focus on
health care, rehabilitation and social inclusion. There will be a
requirement for close co-operation between the provider and the referring
agency to develop a seamless package of care that meets the need of
physical, social and psychological care, security, ongoing intervention of
specialist services and community integration. There will be a range of
therapeutic interventions offered, both on an individual and group basis.
These will include management of complex health needs, rehabilitation,
behaviour management and trigger recognition, and basic living skills. In
meeting such objectives there is also a requirement to provide reasonable
and realistic visiting access for relatives and monitoring healthcare
professionals.
